What are Liver Cancers?

Liver cancer, also known as hepatic cancer, originates in the liver, an essential organ for metabolizing nutrients, detoxifying harmful substances, and producing important proteins. Liver cancer can be primary (starting in the liver) or secondary (spreading to the liver from another part of the body).

Types of Liver Cancer

1. Primary Liver Cancer

Hepatocellular Carcinoma (HCC): The most common type of primary liver cancer, accounting for about 75% of cases. It originates in hepatocytes, the liver's main functional cells. It often develops in the context of chronic liver disease, such as cirrhosis or chronic hepatitis B or C infection.

Cholangiocarcinoma (Bile Duct Cancer): Cancer that starts in the bile ducts, which are the tubes that carry bile from the liver to the small intestine. It accounts for about 10-20% of primary liver cancers and can be intrahepatic (within the liver) or extrahepatic (outside the liver).

Hepatoblastoma: A rare type of liver cancer that primarily affects children, typically under the age of 3. It originates from immature liver cells.

Angiosarcoma and Hemangiosarcoma: Rare, aggressive cancers that arise from the blood vessels of the liver.

2. Secondary Liver Cancer

  • Metastatic Liver Cancer: Refers to cancer that has spread to the liver from other parts of the body, such as the colon, breast, or lung. This is more common than primary liver cancer.

Risk Factors

  • Chronic Liver Disease: Conditions like cirrhosis (from alcohol use or hepatitis infections) and non-alcoholic fatty liver disease increase the risk of HCC.
  • Hepatitis B and C Infections: Chronic viral hepatitis significantly raises the risk of developing liver cancer.
  • Alcohol Consumption: Heavy and prolonged alcohol use can lead to cirrhosis and increase the risk of liver cancer.
  • Obesity and Diabetes: Can contribute to liver disease and increase cancer risk.
  • Aflatoxin Exposure: Consumption of contaminated food (especially grains and nuts) can increase risk, particularly in certain regions of the world.
  • Family History: Genetic predisposition can also play a role in increasing the risk of liver cancer.

Symptoms

Symptoms of liver cancer can be vague and may include:

  • Abdominal Pain: Especially in the upper right side.
  • Unexplained Weight Loss: Significant loss of weight without a clear cause.
  • Loss of Appetite: Reduced interest in eating.
  • Jaundice: Yellowing of the skin and eyes.
  • Nausea and Vomiting
  • Swelling or Ascites: Fluid accumulation in the abdomen.
  • Fatigue: General feeling of tiredness.
  • Fever: Especially in advanced stages.

Diagnosis

Diagnosis typically involves several methods:

  • Imaging Tests: Ultrasound, CT scans, and MRI can help identify liver tumors and assess their size and spread.
  • Blood Tests: Liver function tests, alpha-fetoprotein (AFP) levels (which can be elevated in HCC), and tests for hepatitis B and C.
  • Biopsy: A sample of liver tissue may be taken to confirm the presence and type of cancer, often using a needle guided by imaging techniques.
  • Laparoscopy: A minimally invasive procedure using a thin tube with a camera to look inside the abdomen and obtain tissue samples if needed.

Treatment

Treatment options depend on the cancer type, stage, and overall health. They may include:

Surgery:

  • Partial Hepatectomy: Removal of a portion of the liver if the cancer is localized and the liver function is adequate.
  • Liver Transplant: Replacement of the entire liver with a healthy one from a donor, typically used for patients with early-stage HCC and significant liver disease.

Ablation Therapy:

  • Radiofrequency Ablation (RFA): Uses heat to destroy cancer cells.
  • Cryoablation: Uses extreme cold to destroy cancer cells.

Embolization Therapy:

  • Transarterial Chemoembolization (TACE): Delivers chemotherapy directly to the tumor while blocking its blood supply.
  • Transarterial Radioembolization (TARE): Delivers radioactive particles to the tumor via blood vessels.

Chemotherapy: Systemic treatment to kill cancer cells, often used when cancer has spread.

Targeted Therapy: Drugs that specifically target cancer cells or their growth pathways, such as sorafenib or lenvatinib.

Immunotherapy: Uses medications to boost the immune system to better recognize and attack cancer cells.

Prognosis

The prognosis for liver cancer depends on factors such as the cancer stage, type, liver function, and overall health. Early-stage cancers and those that are amenable to surgical resection or transplantation generally have a better prognosis. Advanced cancers may require a combination of treatments to manage symptoms and extend survival.
